Latest Shared Care Record news
Here you can find the latest news on the Shared Care Record in Cambridgeshire & Peterborough...
Royal Papworth Hospital NHS Foundation Trust (RPH) is our latest partner to go live with new data feeds into the Shared Care Record. RPH is now sharing admissions, discharge and transfer (ADT) data for service users across the Cambridgeshire and Peterborough ICS area to access.
For the first time, clinicians and other health and social care staff using the Shared Care Record across the region will be able to see this valuable information and how it can improve the care and services they provide. Royal Papworth Hospital is the UK’s leading heart and lung hospital treating around 50,000 patients each year.
The ADT process is one of the most essential hospital workflows and stores valuable patient information about the person and their journey from arrival at the hospital until departure. The transfer can be either transfer to another hospital or service provider, discharge or death.
The availability of ADT information from RPH means that users accessing the Shared Care Record from other service providers will be able to see if their patient has previously been admitted to this specialist hospital for treatment. This will allow health and social care professionals to make informed decisions about the care and support they can offer a person.
As a specialist hospital, RPH, provide services to people across the Cambridgeshire and Peterborough area and beyond. Sharing patient information with other health and social care professionals who are treating them in other settings is therefore particularly important.
The Cambridgeshire and Peterborough Shared Care Record electronically connects patient information from GPs, local hospitals, social care, community, and mental health teams across our area, helping to make care better and safer.
