Toolkit: Your Healthier Future
We’d love your support to promote Your Healthier Future, a new two-year programme to support local people to reduce their risk of Cardiovascular Disease (CVD), made possible with public health funding from Cambridgeshire County Council and Peterborough City Council. Please use this toolkit to encourage local people to participate in the programme, so that we can help our communities reduce their risk and stay healthier for longer.

An exciting new initiative known as Your Healthier Future has been launched by the local NHS to help people in Cambridgeshire, Peterborough and Royston reduce their risk of cardiovascular disease (CVD). Cardiovascular disease is an umbrella term for heart and circulatory illnesses, including stroke and heart disease. In the first three months since its launch in late July, Your Healthier Future has already seen over 2,100 people take positive action to control their risk.
Heart and circulatory disease is sadly a big cause of serious illness and death in the area – with 25% of all local deaths caused by CVD. With funding from Cambridgeshire County Council and Peterborough City Council, NHS Cambridgeshire & Peterborough have now launched Your Healthier Future. This initiative will help participating GP practices identify those patients who are at higher risk of CVD, to offer them treatment, tools and support to help them live well for longer.
Over the coming months you may receive a message from your GP practice inviting you to take part in Your Healthier Future because your records indicate you could be at higher risk. We want to help local people live longer, healthier lives, so if you are invited to participate, please do take up your invitation and get the support you need to work towards your own healthier future.
